The FTW Holistic Health Survey  [Preparation for The Journey]
Need a mental snapshot of what your health looks like holistically - physically, mentally and spiritually?    Take the FTW Holistic Health Survey.  This Survey will allow you to:
 
  • Have the chance to ask yourself serious questions about the overall state of your health.
  • Engage in a one hour discussion where the only topic is YOU and Your Health.
  • Receive written feedback regarding your holistic health state.
  • Get referrals to medical and holistic healthcare professionals who can address your most pressing health concerns, if needed.
  • Have the opportunity to enroll in FTW's one-on-one holistic health coaching program customized to help you reach your optimal health goals!
 
Instructions for filling out and completing the FTW Survey:
 
1.  Schedule a tentative date for a Survey Session Appointment (SSA) with Qadira Yamini, FTW Holistic Health Coach.
 
2.  The evening before the SSA complete the electronic Survey form by answering all of the questions briefly, but as thoroughly as possible.   You may request a printed form of the Survey. 
 
3. Before submitting the Survey electronically, print a copy of the form for yourself. [If you are filling out a  printed form, please make a copy and bring to the SSA.]
 
4.  After submitting the electronic Survey form you will receive a call to confirm the SSA time and place.  If you have a printed form, you will still receive a phone call before the tentative date to confirm the SSA time and place.

What to expect during the Survey Session Appointment [SSA] -- scheduled appointments will be held for 1 hour in a locale that is comfortable and conducive for confidentiatlity :
 
1.  An opportunity to express all of your health concerns - aches, pains, worries and symptoms.
2.  An opportunity to articulate all of the healthy principles you are currently incorporating or want to incorporate in your life. 
3.  An opportunity for you to hear what you think and are doing about your health. 
4.  An opportunity for you to be heard and listened to deeply.
5.  An opportunity to be in a stimulating interactive discussion about possibilties for your future health and wellness.
 
What to expect from the Survey Session Feedback [SSF]-- the 3 - 4 pages of written SSF will be provided to you within five business days after your SSA and will consist of:
 
1. Positive reinforcement of what is working in your life in regards to diet, exercise, relationships, spirituality, etc. 
2.  Recommendations for changing habits and practices that are not healthly.
3.  Red flags for possible medical conditions you need to address immediately.
4.  Suggestions for referrals to doctors, clinics, alternative practitioners and other holistic resources. 
5.  Suggestions and encouragement for further personal development and empowerment. 
 
 
 
The FTW Holistic Individualized Coaching Program
initially is a three (3) month program that consists of optimizing your personal journey to wellness by aiding you in making appropriate nutritional and lifestyle changes.   
 
As an individual it may be difficult to set health goals and meet them – even those you know are vital to your well-being. However, with a coach who is a giving you unfailing support and holding you accountable to what you promise yourself – your best vision of who you see yourself being and becoming is attainable.
 
During the First Leg of the Journey you will:
1. Create a vision for your optimal health.  
2. Identify core goals that with your coach’s assistance you can reach within a three month period.  
3.  Determine whether you need additional help from healthcare practitioner(s).
4.  Get a better understanding of primary and secondary foods. 
5.  Come to a better understanding of nutritional concepts that can help you eliminate your health challenges.
6.   Identify any emotional issues that may be sabotaging you from becoming and sustaining your best self.
7.   Learn new recipes for healthy foods that taste really good.
8.   Shop for foods with a greater awareness in a whole new way. 
9.   Become aware of all the types of food [energy] you need to stay balanced and in-harmony with your Creator, nature and yourself.
 
                The Footsteps To Wellness Program Format:
  • Two (1) hour individualized sessions monthly for 3 months.
  • Sessions will consist of:  setting goals, developing creative strategies for reaching goals, key nutritional & dietary information customized for your health challenge(s), demonstrations, discussion and assignments.
  • (Some) topics that will be addressed are: primary & secondary foods, deconstructing food cravings, cutting-edge nutritional concepts, weight loss principles, food energetics, substituting healthy foods for unhealthy ones and balancing a love for food with other important priorities in life.
  • Also program partcipants will be able to attend free of charge, special monthly FTW events such as health food store and market tours; group discussions related to health and wellness.  And participants will regularly receive special handouts, articles and small gifts.
